- Photocopy of Car Registration (OR & CR)
- Original Enrolment Registration Form (students)
- 1 pc. current (1x1 or 2x2) ID photo of applicant
- Valid I.D. for Officials, Faculty, Employees

Old Applicants:
- Last Year’s Car Sticker Number
- Original Enrolment Registration Form (students only)
